Job summary

Norfolk & Suffolk Foundation NHS Trust is recruiting a Primary Care Based Mental Health Practitioner to work within NN4 Primary Care Network GP practices in North Norfolk. The role supports 16+ patients with non-urgent mental health needs, embedded in a multidisciplinary team, providing assessments and brief interventions.

This NHS post is jointly funded through ARRS and offers collaboration across NSFT and Primary Care to deliver Place-Based Care.

Qualifications

• Registered professional with relevant registration.
• Practice Assessor or Practice Supervisor training or Practice Educator.
• Ongoing professional development toward degree level (if registration follows diploma).

Responsibilities

• Provide assessments, brief interventions and short-term support for people aged 16+.
• Deliver psychoeducation and self-management strategies.
• Collaborate with GP practice staff and local services to direct patients to appropriate support.
• Promote recovery, wellbeing and prevention of deterioration in local communities.
