Job summary

Thrive Tribe is recruiting a Primary Care Support Officer to support Stop Smoking services across County Durham. You will engage with Primary Care partners and develop referral pathways into the service.

You will build strong links with primary and secondary care, promote the service, arrange practice visits and newsletters, and report on engagement outcomes to leadership. This role offers hybrid working from our County Durham base and a collaborative culture.

Qualifications

• Experience in building relationships with health partners and referral pathways.
• Understanding of health inequalities and engaging underserved groups.
• Ability to navigate the primary care landscape and promote services effectively.

Responsibilities

• Form and maintain relationships with Primary Care organisations and partners.
• Attend networking events and share information on referral routes and eligibility.
• Promote the service to primary/secondary care to maintain high visibility.
• Organise practice visits/events and produce a primary care newsletter.
• Develop efficient referral routes by working with primary and secondary care.
