Job summary

The Confederation, Hillingdon CIC in Northwood is seeking a full-time Clinical Pharmacist to join the North Connect PCN on a permanent basis. The role works at Carepoint Practice under supervision of a clinical supervisor and GP team.

You will run repeat prescription processes, manage medicines on transfer of care, and run patient-facing clinics to optimise therapy and promote safe prescribing. The post emphasizes collaboration across the PCN and adherence to NICE guidelines.

Qualifications

• Undergraduate degree in pharmacy with GPhC registration.
• Minimum of 2 years of experience as a pharmacist.
• Experience with common acute and long-term conditions in general practice.
• Working toward or holding independent prescribing qualification.
• Ability to follow legal, ethical, professional and organisational policies and codes of conduct.
• Involves patients in decisions about medicines and supports adherence per NICE guidelines.

Responsibilities

• Conduct clinical medication reviews in practice and care settings to optimise polypharmacy.
• Manage repeat prescription authorisation and medicine transfer processes.
• Develop clinics for patient review and medicine optimisation with GP oversight.
• Provide patient-facing clinics and telephone support for medicine questions.
• Collaborate with care home staff to improve safety of medicines ordering.
• Reconcile medicines after hospital discharge and ensure continuity of supply.
