Job summary

Hope Citadel Healthcare CIC is seeking an experienced Clinical Coder to work within a primary care setting. The role involves reading patient correspondence, coding entries with SNOMED codes, and triaging for GP input to optimise patient care across our network of practices.

The successful candidate will reduce administrative workload for GPs by efficiently processing electronic documents, coordinating with clinicians, and maintaining accurate patient records in our clinical system across

Qualifications

• Read incoming patient correspondence and file into the clinical system
• Code the entries using SNOMED codes
• Triage to determine further action and relay to the GP or team
• Role reduces GP workload and streamlines patient care

Responsibilities

• Downloading electronic documents into patient records
• Accurately coding documents including diagnostic test results and follow-up
• Triaging documents to determine if GP input is required
• Sending tasks to GPs and other healthcare professionals via the clinical system
• Accessing scanned documents and coding/triaging as outlined for electronic records
• Liaising with practice staff and secretaries via the clinical system
• Monitoring outstanding documents and tasks and ensuring timely action
• Aiming to clear all electronic documents within one working day
• Participating in monthly audits
• Attending practice meetings
• Liaising with hospitals, NHS 111 and Out-of-Hours providers as needed
