Job summary
An exciting opportunity has arisen for aproactive and motivated pharmacist with broad clinical knowledge and a goodunderstanding of medicines optimisation to join our team. This is a new seniorrole, which has been created with a dedicated remit to support cost-effectiveprescribing and medicines decisions, whilst optimising use of our digitalsystems to deliver cost savings across the Trust. You must be able to workcollaboratively to deliver the highest quality of care, and to encourage thedevelopment of others, the service and yourselves. It will involve travelacross Herefordshire and Worcestershire. Currently there are no weekendduties, bank holiday working or late nights.You may be expected to work some Saturday mornings in the future. Main duties of the job
You will need to be a confident decisionmaker, with clear leadership skills, excellent pharmaceutical knowledge and asound understanding of pharmacy service and hospital service. The post holderwill
Deliverpharmaceutical care to patients in the Trust; recommend, substantiate, andcommunicate best therapeutic options for patients and implement changes inprescribing practice to ensure evidence-based medicine is followed asappropriate.
Support themedicines optimisation agenda at individual clinical service level. Thisincludes the provision of clinical service level medicine usage reports and tocontribute to policy and protocol development to support the safe andcost-effective use of medicines
Work as part ofan integrated team supporting prescribers across the Trust in optimising theuse of medicines and other prescribable produces.
Be responsiblefor the development and delivery of a medicines optimisation plan for theTrust.

- Masters degree in Pharmacy or pre 2000 equivalent
- Diploma in Clinical Pharmacy or equivalent demonstrable experience
- Evidence of extensive post registration study/CPD
