Job summary

Chase Medical seeks a Prescribing Clinical Pharmacist for a 12-month fixed-term position in Huddersfield, West Yorkshire. The role offers flexible hours around personal life and strong GP support within a high-QoF training practice.

Qualifications

• Provide clinical medication reviews for patients with long-term conditions.
• Lead medicines safety, audits, and antimicrobial stewardship initiatives.
• Run pharmacist-led clinics (face-to-face and remote) as required.
• Support QoF, IIF and PCN medicines-related objectives.

Responsibilities

• Manage acute and repeat prescription requests with safe, effective prescribing.
• Support long-term condition management and medicines optimisation.
• Act on medicines-related tasks from total triage and online consultations.
• Lead on medicines safety, audits, and MHRA alerts.
