Job summary

The NHS North Of England Commissioning Support Unit is seeking a Clinical Pharmacist to join Cruddas Park and Hillsview Surgeries in Newcastle. This part-time role (22.5 hours/week) focuses on clinical pharmacy and medicines optimisation within a GP practice.

You will conduct Structured Medication Reviews, manage long-term conditions, and support safe prescribing as part of a multidisciplinary team, with CPD continuing and opportunities for professional development.

Qualifications

• Must have MPharm degree and CPD ongoing.
• Experience in primary care pharmacy is desirable.
• Ability to deliver Structured Medication Reviews and contribute to medicines optimisation.

Responsibilities

• Provide expert medicines advice to the multidisciplinary team.
• Conduct Structured Medication Reviews and medication optimisation reviews.
• Manage patients with long-term conditions including CKD, heart failure and gout.
• Support medicines reconciliation after hospital admissions and outpatient appointments.
• Aid in achieving local and national prescribing quality targets.
• Undertake medicines optimisation and quality improvement projects.
• Support vulnerable patients and reduce barriers to accessing health care.

Job responsibilities

The Role

This practice-based role is for 22.5 hours per week.

Your week will be varied and typically split between:

Planned Clinical Sessions

• Structured Medication Reviews (SMRs).
• Long-term condition management.
• Patient-initiated medication reviews.

Duty Pharmacist Sessions

• Reviewing acute prescription requests.
• Responding to medication queries from patients and clinicians
• Supporting safe prescribing across the practice.
• Working collaboratively with colleagues to resolve medication-related issues.

You’ll receive regular support from our Lead Pharmacist and GP Clinical Supervisor, with dedicated pharmacy and practice meetings for case discussion, learning and professional development.

Main Duties

• Provide expert medicines advice to the multidisciplinary team.
• Conduct Structured Medication Reviews and medication optimisation reviews.
• Manage patients with long-term conditions including:
  • Chronic Kidney Disease
  • Heart Failure
  • Gout
• Support nursing colleagues with the management of diabetes and respiratory conditions.
• Review acute prescription requests.
• Support medicines reconciliation following hospital admissions and out patient appointments.
• Help the practice achieve local and national prescribing quality targets.
• Undertake medicines optimisation and quality improvement projects.
• Support vulnerable patients and help reduce barriers to accessing health care and medication.
• Maintain awareness of CQC and QOF medication safety requirements.
