Job summary

Story Terrace Inc. seeks a Practice Nurse for Carrfield Medical Centre in Sheffield. The role is permanent, part-time (3 days), and focuses on delivering high-quality nursing care, chronic disease management, and health promotion within a multidisciplinary team.

You will perform patient assessments, chronic disease clinics, immunisations, and home visits when needed, while maintaining accurate records and contributing to QOF targets.

Qualifications

• Registered Nurse with current NMC registration.
• Experience in primary care and long-term condition management.
• Experience delivering immunisations and vaccinations.
• Strong communication and IT skills; able to work independently.
• Competent in using electronic clinical systems including EMIS.

Responsibilities

• Provide high-quality nursing care within scope of practice.
• Undertake patient assessments, treatment, and evaluation of care.
• Maintain accurate and contemporaneous clinical records.
• Identify risks and elevate concerns appropriately.
• Undertake home visits when clinically appropriate.
• Lead nurse-led clinics for long‑term conditions and chronic care.
