Job summary

The Betsi Cadwaladr University Health Board invites applications for a Practice Nurse at Tywyn Health Centre to join a growing primary care team delivering multi-disciplinary health services to Tywyn and surrounding areas.

You will carry out a wide range of nursing duties in primary care, including wound care, immunisation, cervical screening, long-term condition management and minor procedures, working with GPs and allied health professionals. Welsh language ability is desirable.

Qualifications

• Registered first level nurse with active statutory registration.
• Experience in GP practice nursing and long-term conditions.

Responsibilities

• The post holder will perform tasks prescribed and delegated by the GPs and Senior Nurses, working unsupervised to Patient Group Directions, agreed protocols, within an evidence base and within defined competencies.
• The main duties of the role involves undertaking chronic disease management clinics to support QoF (Asthma, COPD/Spirometry, Diabetes and CHD).
• Identifying and managing treatment plans for patients at risk of developing long-term conditions, performing basic nursing tasks (ear syringing, blood pressure monitoring, ECGs), wound care and leg ulcer management, vaccination programmes, and cervical cytology screening; taking pathology samples; assisting GPs with minor surgery; supporting patients in lifestyle changes.
