Job summary

Betsi Cadwaladr University Health Board (BCUHB) is seeking a Practice Nurse to join the Tywyn Health Centre team. You will deliver a wide range of primary care nursing duties, including wound care, immunisation, cervical screening and long-term condition management, working with GPs and a multidisciplinary team.

The successful applicant will have experience in primary care and long-term condition management, with Welsh language ability being desirable and a commitment to equality and diversity.

Qualifications

• Registered first level nurse with active statutory registration.
• Membership of a professional body.

Responsibilities

• The post holder will perform tasks prescribed and delegated by GPs and Senior Nurses, working unsupervised within defined competencies.
• Undertake chronic disease management clinics to support QoF (Asthma, COPD, Diabetes, CHD) and manage at-risk patients.
• Provide wound care, vaccinations, cervical screening, blood pressure monitoring, ECGs, and minor surgery assistance as part of primary care.
