We are looking for a friendly Practice Nurse to join our team, which comprises 12 nurses and HCAs.
Our nursing team includes many specialist nurses, and we are seeking someone who enjoys core nursing tasks such as dressings, immunisations, smears, checks, and connecting with patients.
Main duties of the job
The Practice Nurse will be responsible for various clinical areas including infection prevention and control, health promotion, chronic disease management, well woman and well man clinics, and supporting the practice management team in reviewing and delivering clinical policies and procedures.

The Beaumont Elms Practice operates across two sites and serves approximately 24,000 patients. We are a growing practice with a team of around 70 staff, some of whom have been with us for over 30 years.
Job responsibilities
The core responsibilities of the Practice Nurse include, but are not limited to:
- Supporting the Infection Prevention Control (IPC) lead with audits, training, and ensuring compliance.
- Assisting in the publication of the IPC annual statement.
- Developing and planning care for frail patients and referring them to the Frailty Matron.
- Assessing patient needs to provide appropriate healthcare, based on evidence-based practice.
- Developing and implementing health promotion and wellbeing programmes.
- Managing and supporting patients with chronic diseases through individual treatment plans.
- Identifying and supporting patients at risk of long-term conditions to prevent adverse health effects.
- Providing routine nursing care, diagnosing, and managing acute and chronic conditions as required.
- Providing wound care, reviewing medications, and advising patients on medication use.
- Coordinating travel medicine services.
- Requesting and processing pathology tests.
- Overseeing chronic disease clinics and referring patients to secondary or specialist care when needed.
- Maintaining accurate clinical records in compliance with legislation.
- Ensuring effective use of SNOMED CT codes.
- Maintaining chronic disease registers.
- Participating in well woman and well man clinics.
- Ensuring compliance with the practice chaperone policy.
- Assisting GPs with minor surgery procedures.
- Prioritising health issues and intervening appropriately.
- Supporting the team in clinical emergencies and mental health assessments.
- Implementing vaccination programmes for adults and children.
- Supporting patients with prescribed and over-the-counter medicines.
- Liaising with external agencies to support vulnerable patients.
- Delegating responsibilities safely within scope.
- Supporting safeguarding and substance abuse policies.

Person Specification
Qualifications
- Registered Nurse (Nursing and Midwifery Council)
- GCSE Maths and English (A to C or equivalent)
Experience
- Wound care, sutures, staples removal
- ECGs
- Venepuncture
- New patient medicals
- Immunisations (routine, childhood, travel)
- Requesting and processing pathology tests, advising patients
Disclosure and Barring Service Check
This role requires a DBS check due to the Rehabilitation of Offenders Act (Exceptions Order) 1975.
Salary: £17 to £22 per hour, depending on experience.
