We seek enthusiastic trainees from Geriatric Medicine, Acute Internal Medicine, Anaesthesia, or other medical specialties to apply for this one-year peri-operative fellowship.

Oxford University Hospitals NHS Foundation Trust (OUH) has an established perioperative medicine service in the acute surgical emergency unit (SEU) and for hip fractures. Over the past five years, our services have expanded to provide consultant-led medical perioperative care to major trauma, elective orthopaedic, and vascular services. We also participate in the TAVI MDT. We focus particularly on older patients undergoing emergency surgery and those with complex co-morbidities. Perioperative consultants come from backgrounds in acute medicine, HDU, renal, and geriatric medicine, supported by medical residents (CMT, FY 1 & 2, or GPVTS).

Main duties of the job

Surgical services

OUH has developed perioperative medicine services into emergency surgery, distinct from elective-focused services. Having run successful pilot programs, we plan to expand into elective surgery over the next five years, starting with our expansion to the elective orthopaedic group later this year.

SEU

In addition to local referrals, the SEU provides a tertiary referral service for complex hepatobiliary cases transferred from neighboring counties.

The John Radcliffe is one of five Major Trauma Centres in the South of England, accepting patients with severe injuries from Oxfordshire and neighboring counties, serving a population of over 3 million. The trauma unit also cares for over 500 hip fractures annually.

Vascular

The John Radcliffe Hospital is the regional hub for vascular services, accepting referrals locally and from neighboring counties. Procedures include all forms of vascular surgery, including endovascular aneurysm repair.

Many surgical services are multisite, with emergency work primarily at John Radcliffe Hospital and elective services at Churchill Hospital and Nuffield Orthopaedic Centre. A smaller volume of work occurs at Horton General Hospital in Banbury.

Perioperative physicians are integral to the surgical department, working collaboratively with surgeons, anaesthetists, and the multidisciplinary team. We provide acute and post-operative care, optimize patients for surgery, and coordinate discharge planning for complex cases. We also teach perioperative medicine to surgical junior doctors, attend surgical governance meetings, and contribute to high-risk decision-making. Our team, including perioperative medicine fellows, has led quality improvement and service development projects in areas like patient blood management, post-operative cognitive dysfunction, delirium, consent, and anticoagulation management. These initiatives have positively impacted quality, patient safety, and reduced length of stay and morbidity.

Detailed job description and main responsibilities

This is an exciting opportunity for three successful applicants to learn from our dynamic perioperative team and help develop our service to deliver high-quality care across surgical specialties. You will gain experience in major trauma, general surgery, and vascular liaison, and undertake service development projects in other surgical areas. You will participate in a new joint medical-anaesthetic pre-assessment clinic starting later this year and attend monthly pre-assessment MDTs with anaesthesia and cardiology. You will contribute to local and national audits and lead a quality improvement project of your choice, mentored by a consultant experienced in quality improvement coaching. If interested, we support obtaining the Certificate, Diploma, or Masters in Perioperative Medicine (UCL), providing time within your job plan for private study.
