Job summary

Cornwall Partnership Foundation NHS Trust is seeking a dedicated Practice Nurse to join our busy general practice team. You will provide a broad range of nurse-led services within a friendly, multi-disciplinary environment that values patient-centred care and clinical quality.

Key responsibilities include chronic disease management, wound care, immunisations and health promotion, with opportunities for further training. A supportive team and ongoing professional development are encouraged.

Qualifications

• Registered Nurse with current NMC registration.
• Minimum one year's experience as a General Practice Nurse.
• Experience in Women’s health, Respiratory disease, Diabetes and Long-term condition management.
• Excellent communication and interpersonal skills.
• Ability to work effectively both independently and as part of a multidisciplinary team.
• Commitment to compassionate, patient-centred care and continuing professional development.

Responsibilities

• Provide high-quality, evidence-based nursing care to patients in a general practice setting.
• Support chronic disease management and health promotion.
• Deliver wound assessment, treatment and ongoing management.
• Carry out venepuncture and clinical observations (BP, BMI, etc.).
• Administer injections, vaccines and other prescribed therapies.
• Support cervical screening, NHS health checks and travel health services.
• Work as part of a multidisciplinary team to ensure coordinated patient care.
