Job summary

Integrated Care 24 (IC24) is seeking a Practice Nurse in Brighton to join a growing primary care team. You will deliver high-quality nursing care, manage long-term conditions, and engage in health promotion and prevention within a evolving service.

You’ll work with GPs and a wide clinical team, contributing to a modern, patient-centred care model while supporting staff development and service improvement.

Responsibilities

• Deliver high-quality nursing care across a broad range of primary care services, both independently and in a multidisciplinary team.
• Support patients with chronic conditions, carrying out assessments and management plans.
• Contribute to health checks, smoking cessation, and preventative care initiatives.
• Assess wounds and support routine wound care and broader primary care procedures (immunisations, travel health, phlebotomy, ECGs).
• Participate in screening and health checks across women’s, men’s, cardiovascular and sexual health areas.
• Collaborate with GPs, Practice Nurses, HCAs, and the wider multidisciplinary team to deliver great patient care.
