Job summary

St James Medical Centre in Croydon is seeking an experienced Practice Nurse to join our clinical team, delivering a broad range of nursing services across two practices in Croydon and New Addington. You will work with GPs and the wider team to provide planned and preventative care for our registered patients.

The role requires clinical expertise, sound judgement, and the ability to work independently while maintaining strong communication with colleagues and patients.

Responsibilities

Job summary

We are seeking an experienced and motivated Practice Nurse to join our clinical team at St James Medical Centre. This role offers an opportunity for a skilled nurse to work within a supportive general practice environment, delivering a broad range of nursing services to our registered patient population across both of our practices in Croydon and New Addington.

The successful candidate will work alongside GPs and the wider practice team to provide planned and preventative care, support patients with long-term conditions and contribute to improving health outcomes within our community.

This is a varied role requiring clinical expertise, good judgement and the ability to work independently while maintaining effective communication with colleagues and patients. The ideal candidate will be committed to delivering excellent standards of nursing care and supporting the continued development of primary care services.

Main duties of the job

The Practice Nurse will provide a range of clinical services to patients registered with St James Medical Centre, working as part of the wider healthcare team to deliver safe and effective care.

The post holder will undertake nursing assessments, develop care plans and support the management of patients with long-term conditions, including diabetes, respiratory disease, cardiovascular disease and hypertension. They will deliver preventative healthcare services including immunisations, cervical screening, NHS health checks and health promotion advice.

Job responsibilities
Clinical Responsibilities
  • Provide safe, effective, and patient-centred nursing care in a primary care setting
  • Undertake long-term condition management clinics including (but not limited to) diabetes, asthma, COPD, hypertension, and cardiovascular disease
  • Perform cervical screening in line with the NHS Cervical Screening Programme
  • Administer childhood and adult immunisations and vaccinations
  • Carry out wound care, dressings, and minor injury management
  • Support health promotion and disease prevention activities, including lifestyle advice and patient education
  • Identify and escalation safeguarding concerns in line with practice policies
  • Maintain accurate, contemporaneous clinical records using EMIS Web

  • Maintain up-to-date professional registration, revalidation, and CPD
  • Participate in clinical supervision, appraisal, and personal development planning

  • Registered Nurse (Adult) with current NMC registration
  • Evidence of ongoing Continuing Professional Development (CPD)

Desirable
  • Previous experience working in an NHS GP practice
  • Experience supporting QOF, enhanced services, or local incentive schemes
  • Experience working within a multidisciplinary or PCN team
  • Experience contributing to clinical audits or quality improvement projects
  • Experience using EMIS Web and AccuRx
  • Sound knowledge of primary care nursing and NHS clinical pathways
  • Ability to deliver evidence-based care in line with national and local guidelines
  • Understanding of infection prevention and control in a GP surgery setting
  • Awareness of safeguarding responsibilities for adults and children
