Background

This is an excellent opportunity for a passionate and experienced Practice Nurse with Chronic Disease Management experience to join a highly regarded GP Surgery. The position offers up to 31.5 hours per week, working within a strong, long-serving MDT and a close-knit Nursing team. The practice prefers candidates with experience in Asthma, COPD, or Diabetes. Salary

£39,000 - £41,850 per annum DOE FTE (£21.46 per hour) + NHS pension + 5 weeks annual leave + Bank Holidays + indemnity
