Background – This is a superb opportunity for a passionate and experienced Practice Nurse with Chronic Disease Management experience to join a highly regarded GP Surgery. The role offers up to 31.5 hours per week, working within a strong and long-serving MDT and nursing team. The practice prefers candidates with experience in Asthma, COPD, or Diabetes. It provides a culture of continuous career progression and development, with ongoing support from the wider practice team. This is an excellent opportunity for an experienced Practice Nurse prioritizing patient care.
Salary – £39,000 – £41,850 per annum DOE FTE (£21.46 per hour) + NHS pension + 5 weeks annual leave + Bank Holidays + indemnity
