Leeds Road Practice is seeking a motivated practice nurse to join our busy Harrogate team. You will deliver evidence-based nursing for long-term conditions and preventative care across our patient population of around 15,000.

The role focuses on chronic disease management, immunisations, wound care, and health promotion, with support from GPs and other healthcare professionals. NMC registration and a Diploma in Diabetes are preferred.

Qualifications

• Practice Nurse experience is required.
• Diploma in Diabetes is a requirement.
• Good interpersonal and communication skills are essential.

Responsabilités

• Chronic disease management of Diabetes, Asthma, COPD and CHD.
• Assist in routine patient care tasks as directed by nursing staff and GP's.
• Cytology.
• Wound care and dressing management.
• Routine immunisations including childhood immunisations.
• Contraception checks.
• Ensure nursing activity aligns with GMS, QOF and services.
• Adhere to NMC code and local policies.
