Job summary

Adam Avenue Practice in Airdrie is seeking a Practice Nurse to join our expanding clinical team. You will work within our health centre with a focus on chronic disease management and patient risk assessment.

The successful candidate will demonstrate excellent communication and team collaboration. Knowledge of Vision and Docman is desirable but not essential; salary is dependent on experience and a permanent contract is offered.

Qualifications

• Must have experience in chronic disease management.
• The right candidate must have the ability to manage risk and potential conflict with excellent interpersonal/verbal communication.
• Knowledge of Vision and Docman is desirable but not essential.

Responsibilities

• Provide nursing care for chronic disease patients.
• Collaborate with GP Partners and health team to support patient care.
