Job summary

Academy Medical Centre in Scotland is seeking a highly motivated Practice Nurse or Nurse Practitioner to join a friendly, supportive team. Open to experienced practice nurses or those starting in General Practice Nursing. Role will develop to suit patient population and practice needs.

• Long term condition reviews using Medlink, telephone and face-to-face methods.
• Maintain accurate and timely record keeping.
• Assess, plan, implement and review care plans with patient wishes and goals.
• Aid management with development and implementation of DES projects.
• Interpret biochemistry and microbiology results.
• Identify patients who would benefit from health optimisation through education and lifestyle changes.
• Collaborate with the wider team to deliver high-quality person-centred care.
• Perform venepuncture, blood pressure monitoring, injections.
• Support nursing and medical students on placement.

We are searching for a highly motivated, enthusiastic and person-centred Practice Nurse or Nurse Practitioner to join our friendly and supportive team at Academy Medical Centre. We are open to applications from those with experience in General Practice and those keen to begin a career in General Practice Nursing.

JOB DESCRIPTION

• Collaborating with the wider clinical and administrative team to deliver high-quality person-centred care.
• Performing core nursing duties such as venepuncture, blood pressure monitoring, subcutaneous and intramuscular injections for a range of conditions.
• Support nursing and medical students whilst on placement.
• You must possess effective communication skills both orally and written

Desirable skills and qualifications

The following are desirable for the role. Candidates should either hold these qualifications and competencies or be willing to work towards achieving them with the support of the practice.

• Cervical cytology
• Qualifications in Diabetes and other long-term conditions (including COPD, Asthma, Cardiovascular Disease, Hypertension and Chronic Kidney Disease), or a willingness to work towards obtaining these.
