Job description

We are looking for an enthusiastic Practice Nurse/Nurse Practitioner/Advanced Nurse Practitioner to join our friendly, forward-thinking, caring and innovative team to rise to provide high quality clinical care, adopting traditional values of patient-centered general practice.

You will be working alongside other experienced Practice Nurses and Advanced Nurse Practitioner, providing the whole range of practice nursing duties. We encourage and support learning and professional development and have access to training from our ICB Training Hub. We provide mentorship and conduct regular supportive teaching and nurse appraisals.

We are a well-established training practice with a strong patient centered ethos, dedicated to delivering high quality patient care.

- Training practice with a strong patient centred ethos (GP registrars and medical students, trainee nurse and multiprofessional education)

- We employ a team of Clinical Independent Prescribing Pharmacists who manage chronic diseases, medicines management and repeat prescriptions, Advanced Nurse Practitioner, nurses and extended primary care team including HCAs, Social prescribers; First Contact Physiotherapists, paramedics, Physician Associate

- We are an integral part of a close, forward-looking and innovative PCN

Main duties of the job

Key Responsibilities

  • Carry out routine nursing tasks related to patient care as indicated by patient need including:
  • Cervical Cytology
  • Wound care
  • Removal of Sutures/ clips
  • Routine immunisations/Childhood immunisations
  • Injections e.g. B12, Goserelin and others
  • Urinalysis
  • Travel Advice and Vaccinations
  • Long term conditions, including:
  • Asthma and COPD Management
  • Hypertension Management / 24 hour ambulatory BP
  • Diabetes Management
  • ECGs
  • Chaperoning and assisting patients where appropriate who are being examined by another clinician
  • Requesting pathology tests, for example urine culture, swabs
  • Following agreed clinical protocols.
  • Maintaining and cleaning equipment used by the nurses and GPs and upholding the standards required for Infection prevention and control.
  • Maintaining GP and Nurses rooms, stocking and rotating items as required
  • Maintain general tidiness and cleanliness of nurses and treatment rooms
  • Supervision of the HCAs
  • Ordering of Health Promotion Literature
  • Ordering of vaccinations to maintain stock levels if required
  • Participation in administrative systems in the practice
  • Maintain accurate records
  • Advise other team members of potential problems or errors within the range of assigned tasks
  • Attend and participate in any practice / clinical meetings monthly.
  • On occasion there may be a requirement to undertake home visits
  • Support in QOF and Enhanced Services relevant to role
  • Any other delegated duties appropriate to the post

Undertake annual training and demonstrate competency in anaphylaxis and resuscitation techniques
