A leading healthcare provider in Worksop seeks a Practice Nurse to deliver high-quality care. The role involves managing chronic diseases, conducting health screenings, and collaborating with a multidisciplinary team. Candidates must be registered nurses with relevant qualifications and experience in primary care.
We are looking for a Practice Nurse to provide high-quality, evidence-based care to patients. This role encompasses direct clinical care, management of specialised clinics, and active participation in infection prevention and control initiatives. The ideal candidate will demonstrate strong clinical judgement, excellent communication skills, and a commitment to continuous improvement.
The Practice Nurse will be responsible for clinical areas such as health promotion, chronic disease management, health prevention, cervical screening, and immunisations.
You will manage the nursing care of patients, ensure up-to-date and accurate clinical records are maintained, and QoF reviews completed.
Your role may include:
IPC, Asthma, Hypertension, Diabetes, COPD, Cervical Smears, Contraceptions, Immunisations, Wound management, ECGs, Travel & other Vaccinations. You will work collaboratively with the wider team, including GP Partners, clinicians, pharmacists, and secondary care organisations.
You may need to visit patients at home or support care home visits.
