Job description

We are looking for a Practice Nurse to provide high-quality, evidence-based care to patients. This role encompasses direct clinical care, management of specialised clinics, and active participation in infection prevention and control initiatives. The ideal candidate will demonstrate strong clinical judgement, excellent communication skills, and a commitment to continuous improvement.

Main duties of the job

The Practice Nurse will be responsible for clinical areas such as health promotion, chronic disease management, health prevention, cervical screening, and immunisations.

You will manage the nursing care of patients, ensure up-to-date and accurate clinical records are maintained, and QoF reviews completed.

Your role may include:

IPC, Asthma, Hypertension, Diabetes, COPD, Cervical Smears, Contraceptions, Immunisations, Wound management, ECGs, Travel & other Vaccinations. You will work collaboratively with the wider team, including GP Partners, clinicians, pharmacists, and secondary care organisations.

You may need to visit patients at home or support care home visits.

We are a single-site PCN with 30,000 registered patients, with an established team of 9 GP Partners and over 80 staff, including pharmacists, pharmacy technicians, nurses, HCAs, management, admin, ACPs, PAs, and ANPs. We also have social prescribers, Care Coordinators, and strong links with community nurses and the voluntary sector.

Job responsibilities

The core responsibilities include, but are not limited to:

Primary Responsibilities

  • Assess patient needs to ensure appropriate healthcare, using evidence-based practice
  • Develop, implement, and evaluate care plans for chronic disease patients
  • Identify and support at-risk patients to prevent long-term conditions
  • Provide routine nursing care following clinical guidelines
  • Diagnose and manage acute and chronic conditions
  • Deliver wound care, ECGs, and other procedures
  • Review medications for compliance
  • Guide patients on medication use and side effects
  • Request and process pathology services
  • Oversee chronic disease clinics and refer to secondary care as needed
  • Maintain accurate clinical records and use SNOMED CT codes appropriately
  • Develop and participate in clinics such as well-woman and well-man
  • Assist GPs with minor surgeries
  • Prioritize health issues and manage clinical emergencies
  • Recognize and refer mental health needs
  • Implement vaccination programs for all ages
  • Support medication management within scope of practice
  • Liaise with external services for patient support
  • Delegate responsibilities safely and appropriately
  • Support safeguarding and substance abuse policies
  • Deliver opportunistic health promotion
