A leading healthcare provider is seeking an experienced Practice Nurse to join their dedicated team in Tonbridge. The role involves managing chronic diseases, providing immunisations, and offering health advice to patients. Candidates should have a formal nursing qualification and be registered with the NMC. We are seeking to recruit an experienced, highly motivated Practice Nurse to join our friendly, innovative team who are committed to delivering high standard of patient care. You will need to have previous experience working in primary care and have a fully rounded set of skills. You will work both independently and as a member of our multidisciplinary team and will be fully supported by clinical and non-clinical staff.
Management of chronic disease -monitoring and development of individual care plans, agreeing these as appropriate with the patient and other health professionals. A special interest in asthma and diabetes is preferred.
Family planning & woman's health including cervical smears.
Travel health advice and vaccinations.
Child immunisations and advice.
QOF, DES & LES Implementation.

MAIN DUTIES AND RESPONSIBILITIES
Provision of health advice to patients.
Undertake immunisations, injections and vaccinations.
Carry out screenings, e.g. lipids, blood pressure, cervical smears, etc.
Provision of support and assistance with long term conditions specialising in diabetes and asthma.
Undertake ECG and other routine monitoring procedures.
Provision of travel advice.
Venepuncture and the taking and preparing of pathology samples.
Provision of general nursing duties, e.g. dressings, ear syringing, etc.
The maintenance of health promotion literature and information for patients.
The inputting of information onto the Practice clinical system.
Use of Practice clinical system for patient searches for clinics and patient recall.
Ensure that infection control procedures are followed.
The ongoing cleaning of clinical equipment and working surfaces.
The maintenance of treatment room equipment, including testing and recording.
