A leading healthcare provider is seeking a highly motivated Practice Nurse to join their team in Tadworth. This part-time role involves delivering high-quality nursing services, including immunisations and chronic disease management, within a supportive environment. The ideal candidate will be a Registered General Nurse, committed to professional development, with the opportunity for full training provided for the right individual.
We are seeking ahighly motivated Practice Nurse who shares our commitment to delivering highquality services to our patients. We are a forward thinking training practicewith GP registrars, placements for medical students from St Georges plusnursing students via our PCN (Primary Care Network). We are CQC rated 'Good'with a high QOF achievement. We are part of a successful Federation supportingextended access provision and other services and work collaboratively with fourother practices as part of the Banstead PCN. Our nursing team is lead by asupportive Practice Nurse Mentor who is motivated to provide a high standard ofcare in the most effective and efficient way with great emphasis on up to dateguidelines and standards. Ideally, we require a fully trained Practice Nurse,however, full training will be provided for the right candidate. This is apart-time vacancy working 15 hours per week.This is an ARRS funded role and as per the National Specification, anyapplicants cannot be currently substantively employed by a Banstead PCNpractice.
We require ourpractice nurses to provide core duties such as immunisations, smears anddressings. Other desirable skills would include warfarin, asthma, COPD anddiabetes. Previous experience of EMIS would be an advantage. If however, youare new to Practice Nursing, full training will be provided for the rightcandidate with the full support of our Practice Nurse Mentor. An informal visitto the practice to meet the team can be arranged. Regular educational and hottopic events are arranged by the PCN and local ICB.
The practice isbased in pleasant surroundings in a semi-rural location, looking afterapproximately 12,000 patients in a modern purpose built surgery with well servedstaff facilities with comfortable 'break' sitting room and small garden. Wepride ourselves on our very happy, cheerful working environment with asupportive and enthusiastic clinical and non-clinical team, with many longstanding member of staff, some of which have left and come back! We haveregular practice and team meetings to keep all staff up to date which sometimesincludes visitors keeping us informed of local services in the area. There iseasy local parking and a train station just a 15 minute walk away with theadded bonus of beautiful Surrey countryside nearby. We have regular socialactivities including an annual team building afternoon and a Christmas party.
To ensure theprovision of comprehensive and supportive nursing care to patients and to workas an effective member of the multi-disciplined team To manage the full rangeof treatment, screening, immunisations etc. within the practice nurse clinicsincluding any identified for the management of agreed and specific chronicdiseases.
General NursingDuties & Responsibilities
To provide nursingservices to patients, including treatment, screening, advice and minortherapeutic procedures.
To undertakediagnostic tests including blood glucose monitoring, PEFR and urine analysisand ECG recording.
To advise on andsupervise patients use of devices including PEFR meters, urinary diagnosticstix, blood glucose monitoring.
To provide childimmunisations on completion of training.
To take and despatchsafely laboratory specimens including blood, urine, swabs and smears.
To providetreatment for cuts and bruises, dressings of wounds and leg ulcers acute andchronic, compression bandaging, Doppler measurements and removal of sutures.
To provide immunisationsfor all ages and diseases and to know the contra-indications and side effectsof vaccinations. Administration of nebulisers.
To provideimmunisations and health advice to patients travelling abroad. (on completionof travel course).
To keep up to datewith resuscitation techniques on an annual basis and other mandatory trainingrequirements.
To provide wellwoman health checks and cervical cytology
To assist GPs withminor surgery procedures.
To act as chaperoneto GPs for examination of patients.
Health Promotion
To give lifestyleadvice to patients including advice on smoking, alcohol consumption, diet andexercise.
To provide newregistration health checks for new patients registering with the practice.
To provide healthadvice to patients concerning risks of heart disease and cancer, includingthose with a personal and/or family history.
To provide othergeneral and specific advice in line with national and local health priorities.
To haveresponsibility for the health, safety and welfare of self and others and tocomply with health and safety regulations.
Chronic DiseaseManagement
To assist thesenior practice nurse in running chronic disease management clinics
To contribute toimplementation of national service frameworks for coronary heart disease,diabetes, asthma, COPD, stroke, family planning, chronic kidney disease,obesity, smoking cessation.
Electronic PatientRecords
Maintain accuratecomputerised and manual records of all patient consultations.
Use of templates,where available, for all consultations to ensure data completeness, continuityand accuracy.
Support thepractice strategy on data collection and entry and performance against the newGMS Quality & Outcomes Framework.
Equipment andSupplies
To help maintainthe nursing and treatment rooms in good order, ensuring equipment is sterilisedand that all sterile procedures are followed.
Maintain records ofstock held and ordering of dressings, drugs, instruments, request forms,specimen bottles, reagents and any other utensils.
To assist in themonthly stock take of vaccines.
Other
To maintain patientconfidentiality at all times
To act inaccordance with the NMC code of conduct and comply with PREP and to comply withthe requirements of the data protection act.
To be responsible forpersonal and professional development including continuing education,consultation skills and clinical standards and to formulate and agree apersonal development plan each year.
To play a full rolein helping the practice in the Practice Development Plan.
To help developteam spirit within both the practice and the Primary Health Care Team. Attendwhere possible Primary Health Care team meetings.
