Job summary

Primary Integrated Community Services is seeking a dedicated nurse to join a busy general practice. The role focuses on respiratory reviews, wound care, and long-term condition management within a strong multidisciplinary team.

You will have avenues for professional development and structured training pathways, with NHS Pension and other leave provisions. You will support patients with chronic conditions, promote self-care, and work closely with GPs, nurses, HCAs, and community teams to deliver

Responsibilities

  • Deliver high-quality nursing care through respiratory reviews, wound care and long-term condition management, supporting patients to stay healthy and engaged in managing their wellbeing.
  • Run independent clinics, review treatment plans and promote effective self-care to improve patient outcomes.
  • Work collaboratively with GPs, nurses, HCAs and community teams to provide safe, evidence-based care and ensure patients receive coordinated support across the multidisciplinary team.

Job description

This role is based within a busy general practice setting, providing high-quality nursing care to a diverse patient population with a strong focus on respiratory long-term condition management, wound care and health promotion. You'll receive ongoing clinical support from the Lead Nurse and GPs, with opportunities to develop specialist skills through structured training pathways such as respiratory upskilling and continuous professional development. As a key member of the multidisciplinary team, you'll work closely with GPs, practice nurses, healthcare assistants and community teams to deliver safe, evidence-based care. You'll support patients with chronic conditions, helping them manage their health, prevent deterioration and improve their overall wellbeing. This is a fully integrated role with a strong support network, clear clinical supervision, and a collaborative team structure designed to help you thrive.,

  • Deliver highquality nursing care through respiratory reviews, wound care and longterm condition management, supporting patients to stay healthy, confident and engaged in managing their own wellbeing.
  • Run independent clinics, reviewing treatment plans, promoting effective selfcare to improve patient outcomes.
  • Work collaboratively with GPs, nurses, HCAs and community teams to provide safe, evidencebased care and ensure patients receive coordinated support across the multidisciplinary team. Experience working in a patient-facing clinical or care environment, ideally involving long-term condition management or primary care.
  • Strong communication and organisational skills with the ability to work independently and collaboratively within a multidisciplinary team. We offer a comprehensive package which includes:
