An NHS GP surgery in Hackney is seeking a dedicated Practice Nurse/Advance Nurse Practitioner. This role involves delivering high-quality patient care, managing chronic diseases, and supporting patient wellbeing through various clinical tasks. Queensbridge Group Practice is a well-established NHS GP surgery in Hackney with approximately 10, 500 registered patients. We are a friendly, supportive, training practice that has a record of good performance against local and national requirements, as well as a committed multi-professional team. We currently use the Emis Web clinical system.
We are looking to recruit a caring and dedicatedPractice Nurse/ Advance Nurse Practitioner, preferably with GP experience. Weare a high performing, patient - orientated and friendly team with a focus onclinical excellence, education and staff wellbeing.
Primary Care experience is desirable; however we will considercandidates with experience in secondary or community care sectors. We are keento support the right candidate in developing their skills via internalmentorship/ external development and offer an opportunity for clinical andmanagerial leadership.
City and Hackney has an active Practice Nurse Community that holdsregular Practice Nurse Forums and is represented by Practice Nurse Directors onthe City and Hackney CCG/ GP Confederation board.

Job responsibilities:
Work as part of the Nursing team to assess, plan, provide and implement and evaluate nursing care to meet the care needs of individuals in the practice population including assessment, planning, delivery and evaluation of:
Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition
Assist in and perform routine tasks related to patient care as directed by GPs
Working alongside the GPs to manage Chronic Disease registers including Asthma, COPD, Diabetes, Heart Disease etc. Maintain disease registers in liaison with administrative staff. Undertake annual review of patients understanding and ability to self-manage.
Ordering of clinical supplies including vaccines
Participation in administrative systems in the Practice
On occasion there may be a requirement to undertake home visits
Any other delegated duties appropriate to the post
