Some experience in chronic disease management and primary care is desirable but not essential, as training and in-place supervision will be provided if required. We prioritize recruiting a nurse who can bring:

About Crickhowell Group Practice

Crickhowell Group Practice is a rural general practice located within the Bannau Brycheiniog National Park in Crickhowell, Powys. We are six miles from Abergavenny, 13 miles from Brecon, and 25 miles from Cardiff, with good local amenities and schools.

The practice has 7 doctors (5 GP partners and 2 salaried GPs), 4 nursing staff, and 2 HCSWs. We also provide services through NHS staff, including health visitors and community mental health. We are a high-achieving practice under the QAIF, consistently receiving high patient satisfaction ratings. We strive to provide the highest standards of care.

Job Responsibilities

Provide a range of nursing care, including:

• Chronic disease programs (diabetes, respiratory, CHD) following training or proven competency
• Venepuncture and ECG
• Opportunistic screening and new patient checks
• Wound care and dressing techniques
• Injections under prescriptions or PGD, with safe storage and disposal
• Monitoring anticoagulation (INR clinic)

Additionally, the role involves:

• Recognizing signs of abuse, violence, substance abuse, and suicidal tendencies, and knowing referral procedures
• Completing mandatory updates annually
• Applying infection control measures
• Supporting development of procedures and contributing to clinical governance
• Participating in practice administration and maintaining professional development
• Contributing to practice targets and maintaining clinical records
• Supporting equipment management and stock maintenance
• Supporting integrated primary care services and risk management
