Key Responsibilities:

• Provide nursing care and support to patients, including chronic disease management, immunisations, and health screenings.
• Providing specialised care for diabetics or respiratory conditions.
• Work collaboratively with GPs and other healthcare professionals to deliver comprehensive patient care.
• Conduct health assessments and develop care plans tailored to individual patient needs.
• Support public health initiatives and health promotion activities.
• Maintain accurate and up-to-date patient records.

Qualifications and Experience:

• Registered Nurse with current NMC registration.
