A well-established medical centre in Cardiff is seeking an experienced practice nurse to join their clinical team. The role involves managing chronic diseases, providing various clinics, and ensuring high-quality patient care. Llandaff North Medical Centre is looking for an experienced practice nurse to join our friendly clinical team, to provide high quality patient care and contribute to the development of services within our well established and thriving practice.
The post has become available for 16-24 hours per week, working across 2 or 3 days: Wednesday, Thursday and Friday. The successful applicant should be flexible.
The role would suit an experienced practice nurse.

The role involves actively managing patients on chronic disease registers. This will include asthma, hypertension, COPD and diabetes. You will be working alongside our nursing team of one practice nurse and one healthcare assistant with support from the wider clinical team.

Llandaff North Medical Centre is a successful training practice located in Cardiff and provides healthcare to the surrounding population of around 6,500 people. The practice was built in 1997 and currently has 8 Doctors (3 partners, 4 salaried and 1 trainee GP), 2 practice nurses, 1 Healthcare Assistant and a range of administrative staff.
The practice also has a range of NHS staff in the form of Health Visitors, Counsellors, Midwives, Pharmacist and community nurses who use our rooms to provide additional services to our patients.
We are a friendly practice with good staff retention and a strong team working culture.
To initiate and maintain disease management and health promotion. To provide general and specific health screening to the practice population (within agreed practice protocols) with referral to general practitioners as necessary. This will be within the professional competence of the post holder.
Working within the nursing and other clinical teams, provide a variety of clinics, always working in accordance with the NMC code of conduct.
