Job summary

Sirona care & health is recruiting a Band 5 Physiotherapist to join the Burren Intermediate Care Beds Service in Bristol and surrounding areas. The role supports BNSSG discharge pathways, rehabilitation, and multidisciplinary care to help patients return home safely.

You will work in a dynamic MDT with therapists, nursing, medical and social care colleagues, managing a defined caseload and contributing to Home First principles and timely discharge planning.

Responsibilities

• Provide a safe, effective, patient-centred and evidence-based Physiotherapy service within the Intermediate Care Beds Service.
• Assess, treat and manage patients requiring rehabilitation, recovery and discharge planning support.
• Work as an autonomous practitioner managing a defined caseload within professional scope of practice.
• Contribute to person-centred rehabilitation programmes that maximise independence and support positive outcomes.
• Work collaboratively within the multidisciplinary team to facilitate safe and timely discharge planning.
• Promote Home First principles and support individuals to achieve their rehabilitation goals.
