Job summary

Sirona care & health is recruiting a Band 5 Physiotherapist to join the Burren Intermediate Care Beds Service, part of the BNSSG discharge and patient flow pathway, providing rehabilitation before returning home.

You will work within a dynamic multidisciplinary team across OTs, nursing, medical, social care and community services, delivering person-centred rehabilitation and supporting timely discharge to the preferred home setting.

Qualifications

• Professional physiotherapist registration and practice.
• Ability to assess and treat rehabilitation needs.
• Ability to manage a defined caseload autonomously.
• Contribution to multidisciplinary rehabilitation programmes.

An exciting opportunity has arisen to join the newly established Burren Intermediate Care Beds Service as a Band 5 Physiotherapist. This innovative service forms a key part of the BNSSG discharge and patient flow pathway, supporting individuals who no longer require acute hospital care but continue to require rehabilitation, recovery support and multidisciplinary intervention before returning home. The service currently provides 32 intermediate care beds alongside a specialist bariatric pathway and is designed to support timely transfers of care, maximise patient independence and improve outcomes through high-quality rehabilitation. As part of a dynamic multidisciplinary team, you will work closely with Occupational Therapists, Nursing colleagues, Medical staff, Social Care professionals and Community Services to deliver person-centred rehabilitation programmes that enable people to achieve their goals and return to their preferred place of residence. This is a unique opportunity to join a developing service during an exciting period of growth and transformation, helping to shape rehabilitation practice and contribute to a model that supports patient flow across the wider health and care system.,

• To provide a safe, effective, patient-centred and evidence-based Physiotherapy service within the Intermediate Care Beds Service.
• To assess, treat and manage patients requiring rehabilitation, recovery and dischargeplanning support.
• To work as an autonomous practitioner managing a defined caseload within professional scope of practice.
• To contribute to person-centred rehabilitation programmes that maximise independence and support positive outcomes.
• To work collaboratively within the multidisciplinary team to facilitate safe and timely discharge planning.
• To promote Home First principles and support individuals to achieve their rehabilitation goals.
