East Lancashire Hospitals NHS Trust is seeking a Band 6 Physiotherapist to join a proactive community rehabilitation team. The role involves autonomous assessment and home-based treatment, supporting discharge from acute settings and long-term condition management.

You'll work across community settings with the MDT, supervise staff, and contribute to a comprehensive training programme. The nominated base is Blackburn with Darwen, with occasional cross-locality work across the East Lancs

Qualifications

  • Post graduate therapy experience including community and rehab.
  • HCPC registration is required.
  • CPD portfolio with recent courses relevant to clinical rotations.
  • Membership of CSP or RCOT preferred.
  • Involvement in supervising colleagues or students.

Job summary

An exciting opportunity has arisen in a community rehabilitation team for a band 6 physiotherapist to join a supportive and enthusiastic community therapy service as a Physiotherapist, working across community delivering rehabilitation in patients own homes following discharge from an acute bed, or supporting Long term condition management.

The team comprises of qualified physiotherapists, and assistant practitioners, and assistant therapy staff, who work closely with the wider MDT and INT teams in community.

You will be working with patients with a large variety of conditions and disabilities, in their own homes and local community. You will be responsible for assessment, intervention, and discharge of a caseload of patients, act as the direct supervisor to staff and provide support and expertise to the other members of the team.

The service operates across 5 days , Monday to Friday

Your nominated base will be Blackburn with Darwen, however cross locality working across the East Lancs footprint at times.
Main duties of the job

The post holder is an autonomous practitioner, responsible for provision ofspecialised assessment and treatment of patientsreferred to the service. . Responsible for managing a caseload of patients with a variety of complexconditions including long term condition management and acute hsopital discharges. You will see patients in own homes , working with an MDT in other organisations. You will carry out specialist assessment and treatment including the prescription of essentialequipment as part of the therapeutic intervention .

You will play an important link with PCN and MDT teams in attending meetings as part of neighbour hood team working

Person Specification
Essential
  • Post graduate therapy experience to include community and rehab
  • Profession specific Diploma/degree in Occupational Therapy or Physiotherapy
  • Health Professions Council Registration
  • Evidence of CPD maintained in a portfolio including attendance at recent postgraduate courses relevant to the clinical fields in rotations.
  • Membership of the Chartered Society of Physiotherapy/ Royal College of Occupational Therapy
  • Involvement in Special Clinical Interest Group
  • Post graduate training in relevant clinical areas
  • Experience of delivering training to colleagues in health and/or other agencies.
  • Experience of working in an integrated therapy or multi professional MDT
  • Experience of therapy student and/or support staff supervision
  • Contribution to clinical education of less experienced staff/students/assistants
  • Experience of working with patients with dementing
