This is a new and exciting team of nurses, physiotherapists, and occupational therapists. The team includes doctors, specialist nurses, and therapists who care for frail and elderly patients in the Emergency Department, the Acute Admissions Unit, and throughout the hospital. The development of virtual frailty wards aims to support patients in their own homes via early supported discharge or admission avoidance processes.
The Frailty Virtual Ward at Newham Hospital provides safe care at the patient’s home to avoid hospital admission or facilitate safe, earlier discharges for patients requiring ongoing management with clinical and care support in their residence. The unit aims to improve patient experience and outcomes.
The Senior Physiotherapist is a member of the virtual frailty ward team, delivering multidisciplinary assessment and clinical management for frail patients within the hospital, emergency department, and community. The post holder assesses, plans, implements, and evaluates care for patients with frailty needs, making necessary adjustments.
