Job description

About Altais

At Altais, we’re on a mission to improve the healthcare experience for everyone—starting with the people who deliver it. We believe physicians should spend more time with patients and less time on administrative tasks. Through smarter technology, purpose-built tools, and a team-based model of care, we help doctors do what they do best: care for people.

Altais includes a network of physician-led organizations across California, including Brown & Toland Physicians, Altais Medical Group Riverside, and Family Care Specialists. Together, we’re building a stronger, more connected healthcare system.

About the Role

We’re a collaborative, purpose-driven group that’s passionate about transforming healthcare from the inside out. At Altais, we support one another, adapt quickly, and work with integrity as we build a better experience for physicians and their patients.

Altais Medical Group is a growing team-based primary care model, led by physicians. Our practices deliver care that is centered around their patients, built on clinical expertise and enriched by technology to help patients get the care they need when it matters.

This role provides clinical primary care performance by a physician specifically trained and skilled in comprehensive first contact and continuing care for persons with any undiagnosed sign, symptom, or health concern not limited by problem origin (biological, behavioral, or social), organ system, or diagnosis.

The position works closely with the clinical team and is primarily responsible for managing direct patient care, implementing evidence-based medicine, and ensuring adherence to national and internal practice guidelines and policies. The physician also provides patient advocacy to achieve cost-effective care through coordination of health services, promotes effective communication, and plays a key role in driving clinical initiatives for BTHealth in collaboration with other health professionals.

You Will Focus On

  • Disease prevention
  • Health maintenance
  • Counseling
  • Patient education
  • Annual Health Assessments (AHA)
  • Chronic Care Management (CCM)
  • Providing direct and indirect patient care as part of the treatment regime, identifying actual or potential risks based on patient screening data.
  • Collaborating with providers, ancillary personnel, community resources, and interdisciplinary team members to proactively resolve patient needs.
  • Leveraging the use of the Electronic Health Record (“EHR”) to manage and improve patient care, workflows, and clinical compliance.
  • Advocating for patients to accomplish cost-effective care through coordination of health services.
  • Promoting effective communication with patients and encouraging their role as a partner in health care.
The Skills, Experience & Education You Bring
  • Providing patient-centric care and serving as a patient health advocate.
  • Working in interdisciplinary teams and employing evidence-based practice.
  • Applying quality improvement principles.
  • Ability to function independently as well as part of a team in a dynamic environment and adapt quickly to changes.
  • Excellent communication skills with the ability to adjust styles when working with patients, families, and colleagues.
  • Highly organized, strong time management, and flexibility to manage multiple timelines and activities.
  • Minimum of 1–2 years clinical experience.
  • Successfully completed residency in their specialty.
  • Preferred: 3+ years professional experience working as a PCP.
  • Significant experience working with an EHR, preferably Epic.
