Job description

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time opportunity providing home health visits from our Williston, VT office. This office serves adult and geriatric clients across Orange County. Candidates must have at least one year of clinical experience as a licensed Physical Therapist. As a home care Physical Therapist, you will be part of a multidisciplinary team delivering rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities include:

  • Make home visits to clients in designated areas.
  • Perform diagnostic tests and measurements related to mobility, strength, balance, and safety.
  • Develop and implement individualized care plans with therapeutic exercises, gait training, and other interventions.
  • Assess and update care plans, collaborating with nursing staff.
  • Educate clients and families on rehabilitative activities to promote health and independence.
  • Document observations and interventions accurately using electronic devices.

Qualifications:

  • Vermont Physical Therapist license.
  • Minimum one year of recent PT experience.
  • Graduate of an accredited Physical Therapy program.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal and organizational skills.
  • Proficiency with computers; EMR experience preferred.
