General Description Of Position:

Under supervision of a Licensed Physical Therapist, provides Physical Therapy services to clients of Your Choice Health Care, LLC.

Essential Functions:

• Assists in the practice of physical therapy by helping to improve or maintain functional strength, movement, and mobility.
• Assists the Licensed Physical Therapist in collecting data for evaluations and progress monitoring.
• Keeps the Licensed Physical Therapist informed of all relevant client information.
• Completes documentation promptly according to agency procedures.
• Summarizes, records, and reports data to the Licensed Physical Therapist as needed.
• Maintains an efficient schedule of services to clients.
• Prepares and maintains equipment and supplies.
• Safeguards confidential information.
• Maintains continuing education in the related field.

Knowledge, Skills, and Abilities:

Knowledge of service delivery models in home health, therapy practices, and safety protocols. Ability to interpret policies, follow instructions, communicate effectively, maintain records, collect data, prepare reports, and work effectively with staff and clients. Equipment operated includes computers and therapy-related equipment.

Job Details:

• Type: Full-time, Part-time
• Salary: $35.00 per hour
