Physical Therapist Responsibilities

• Conduct thorough physical therapy evaluations to assess residents’ functional abilities
• Design and implement customized therapy plans to restore mobility, strength, and independence
• Deliver effective, evidence-based physical therapy treatments
• Educate residents, families, and staff on mobility, safety, and fall prevention
• Recommend and train in the use of mobility and assistive devices
• Collaborate closely with occupational therapists, speech therapists, nursing staff, and others to ensure comprehensive care
• Maintain accurate and timely documentation that reflects quality physical therapy care
  
  

Qualifications

• Licensed Physical Therapist in the state of Wisconsin (or eligible)
• Strong clinical judgment and communication skills
• A commitment to patient-centered care and quality outcomes
• Previous experience in geriatric or senior care settings preferred, but not required
