An exciting opportunity has arisen to recruit a motivated Physical Health Support Worker to work across Dorset. The main role involves conducting annual physical health checks for individuals diagnosed with a serious mental illness (SMI), including physical observations such as blood samples and ECGs. The role also includes health promotion, interventions, and signposting to appropriate services, with a focus on setting health improvement goals.

The successful applicant will build relationships with patients who may face challenges accessing support, requiring innovative approaches to physical health checks. They should be committed to improving the physical health outcomes for people with SMI and supporting their access to necessary interventions.

The role requires working autonomously under supervision and collaboratively within a busy mental health team. Responsibilities include managing comprehensive physical health assessments, collaborating with care coordinators and primary care services, and supporting individuals in developing self-management plans for their physical health. The role also involves engaging with clinicians, attending relevant meetings, and ensuring appropriate referrals and follow-ups.
