NHS Jobs in Swansea area seeks a Pharmacy Technician to join Llansamlet Surgery. You will help ensure safe, effective medicines use, support QAIF/QOF initiatives, and collaborate with GPs, nurses and pharmacists to improve patient care.

The role focuses on medicines reconciliation, repeat prescribing, and medicines management within the GP practice, with emphasis on governance and patient safety.

Qualifications

• Registered Pharmacy Technician with the GPhC and good practical experience.
• Evidence of CPD and ongoing professional development.
• Knowledge of medicines management and patient care.

Responsibilities

• Undertake medicines reconciliation after hospital discharge and transfers.
• Support repeat prescribing processes and ensure safe prescribing.
• Assist with medicines optimisation and adherence to local guidelines.
• Monitor high‑risk medicines and respond to safety alerts.
• Provide patient information and signpost to relevant services.
• Contribute to audits and quality improvement projects.
• Maintain accurate patient records using Practice systems.
