Job summary

Sheffield Children’s NHS Foundation Trust is seeking a skilled practitioner to provide specialist palliative and end-of-life care for children and young people. You will plan, deliver and evaluate care, coordinating with the multidisciplinary team to meet the needs and wishes of families.

You will support nursing colleagues, offer telephone and in-person care, and participate in service improvements while upholding CARE values and NHS standards.

Qualifications

• RN degree or diploma is essential.
• Current NMC registration is essential.
• Relevant post-registration qualification for the specialty is required.
• 998 or mentorship/supervisor training is desirable.

Responsibilities

• Assess, plan and review specialist complex care for children and young people with life-limiting conditions.
• Deliver care through nurse-led clinics, telephone support and home visits.
• Monitor patients and respond to changes in clinical need.
• Provide specialist education and training to patients, families and healthcare professionals.
• Coordinate care with multidisciplinary teams and external agencies.
• Provide out-of-hours telephone support for end-of-life care.
• Undertake out-of-hours visits to support care needs.
• Maintain accurate clinical records and contribute to service improvement.
