West Leeds Primary Care Network is seeking a qualified nurse to work mainly at Manor Park Surgery with 80% of time on site and 20% PCN duties across six member practices. A driving licence is essential due to travel between sites.

The role offers varied responsibilities including long-term condition management and immunisation delivery. The successful candidate will be NMC registered, hold a nursing diploma/degree, and may be an independent prescriber.

Competenze

• NMC registration or equivalent is required.
• GCSEs in English and Maths (A–C) are mandatory.

Mansioni

• Monitor and manage long-term conditions.
• Assess, plan, implement and evaluate care plans.
• Deliver vaccination and immunisation programmes as applicable.
• Provide health promotion and self-care support to patients.
• Work across Manor Park Surgery and PCN sites and travel as required.
