Primary Care Dietitians Ltd is recruiting an experienced dietitian to join a PCN in Hertfordshire, delivering specialist dietetic services within GP practices. You will manage a clinical caseload, work autonomously within the practice team, and support patients with pre-diabetes, type 2 diabetes, weight management and frailty through person-centred care.

You will contribute to service development, audits and pathway improvements, with opportunities for professional development and protected CPD

Qualifications

• Fully HCPC and BDA registered practising Dietitian.
• Minimum 3 years of clinical experience in dietetics.
• Up-to-date understanding of Health Education England's Roadmap to Practice.

Responsabilités

• Manage own clinical caseload autonomously within GP practices.
• Support patients with pre-diabetes, type 2 diabetes, weight management and frailty.
• Contribute to service improvement, clinical audit and pathway development.
• Deliver group education sessions as needed.
