Qualifications

Job Summary

Location: Across 3 sites within close proximity

Contract: Fixed-term

Hours: TBC

Fednet WTE CIC supports the Primary Care Networks across the borough to recruit Clinical Pharmacists. The Clinical Pharmacist will play a key role in supporting the delivery of the new Network Contract DES Service specifications. They will be an independent prescriber with a CPPE qualification, working across 3 sites. Responsibilities include tackling over-medication, conducting structured medication reviews, and supporting NHS-led programmes such as medicines optimisation and low priority prescribing. Residents in Enhanced Health in Care Homes will benefit from regular clinical-pharmacy-led medicines reviews.

Main duties include:

  • Working as part of a multidisciplinary team in a patient-facing role to assess and treat patients using their medicines expertise.
  • Upholding a prescribing qualification and collaborating with the multidisciplinary team across the Primary Care Network.
  • Managing care for patients with chronic diseases, conducting medication reviews, especially for the elderly, care home residents, and those with multiple long-term conditions.
  • Additional tasks within the scope of the role.

Job Responsibilities

  • Working hours: Monday to Friday, days/times to be agreed.
  • Banding: Between Band 7 to 8a depending on experience.
  • Travel between 3 nearby sites.
  • Providing expertise in medicines management, supporting public health and social care needs.
  • Leading medicines optimisation, antimicrobial stewardship, and quality improvement initiatives.
  • Conducting structured medication reviews to optimize patient medication use.
  • Supporting integration with wider healthcare teams to improve patient outcomes and manage workload.
  • Developing relationships with other pharmacists and healthcare professionals.
  • Engaging in shared care protocols, clinical research, and liaison with specialist pharmacists.

Other Duties

  • Working within policies and maintaining health and safety standards.
  • Promoting client involvement and participating in supervision, training, and meetings.
  • Maintaining tidy administration systems and workspace.
  • Flexibly working to meet client needs and undertaking other appropriate duties.

Person Specification

Qualifications:

  • Degree in pharmacy, GPhC registration, CPPE, and IP qualifications.
  • Willingness to work across 3 sites.

Experience:

  • Minimum two years as a practicing pharmacist, demonstrated in a practice portfolio.
  • Experience with common acute and long-term conditions seen in general practice.

Additional Requirements:

  • Disclosure and Barring Service (DBS) check required.
  • Current UK professional registration essential.
