Grovelands Medical Centre is seeking an enthusiastic Practice Clinical Pharmacist with General Practice experience to collaborate closely with our practice teams, aiming to deliver our pharmacy vision and provide excellent pharmacy-led services for registered patients. Experience in General Practice is desirable.
Requirements include:
- Registration with the GPHC
- Degree or equivalent in pharmacy
- Previous experience in a primary care setting
- Clinical Diploma
Main duties of the job
- Maintain up-to-date clinical knowledge, understand primary care prescribing, conduct medication reviews, and develop strategies for improving prescribing.
- Uphold NHS commitments to addressing health inequalities and ensure care accessibility for all communities.
- Recognize personal and colleagues' limitations, referring to others when necessary.
- Work effectively in a devolved, flexible, autonomous environment.
- Exhibit excellent communication skills, influence others, and manage clinical caseloads in primary care, both face-to-face and remotely.

- Manage repeat prescriptions, acute requests, and medicines transfer processes such as discharge letters and medication reconciliation.
- Provide telephone clinics for patient queries about medicines.
- Perform structured medication reviews and manage aspects of the QOF and enhanced services.
- Assist in developing systems for safer prescribing and foster relationships with local pharmacies and the PCN pharmacy team.
- Participate in meetings to review prescribing and safety issues, and engage in schemes like PINCER and QOF audits.
