Job Summary

We are looking for 2.5WTE PCN Care Coordinators with excellent communication and administrative skills to work with GPs and other clinicians within the PCN. You will assist patients in managing their care, supporting personalized care plans, and engaging them in training and employment to improve wellbeing.

Main Duties
  • Support patients in preparation for or following clinical conversations with primary care professionals.
  • Work closely with GPs and other primary care staff to manage caseloads and support patient needs.
  • Coordinate clinics, liaise with schedulers, and contact patients for appointments.
  • Document and monitor patient coordination and service delivery, supporting data collection and audits.
  • Recognize and respond to emergencies or sudden deteriorations, alerting the team as necessary.
  • Support national screening, immunization programs, and other health initiatives.
  • Coordinate with multi-agency teams and refer patients to social prescribing services.
  • Act as the first point of contact for patients regarding their care, helping them manage needs and access services.
  • Support people in accessing self-management education, peer support, and personal health budgets.
  • Lead and support MDT meetings, liaise with care homes and key providers.
  • Maintain confidentiality and uphold data protection policies.
Person Specification

Essential Qualities: Organisation, teamwork, effective communication, negotiation skills, emotional resilience.

Skills & Knowledge: Knowledge of personalized care, proficiency in MS Office, problem-solving skills.

Experience: Healthcare or NHS experience, data collection, partnership working, general practice familiarity.

Qualifications: ECDL or equivalent, relevant diplomas or NVQ Level 2 Business Administration, commitment to professional development.

Additional Information

This role is subject to DBS clearance and requires access to transport for travel across the locality.
