Job overview

An exciting opportunity has arisen for a Patient Pathway Co coordinator within the Acute Medicine POD at Newham General Hospital.

The post holder will be the point of contact for all administrative issues relating to patients' pathway of care. The post holder will work as part of a multi-disciplinary team and will provide support in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly. The post holder will ensure high levels of patient and clinician satisfaction by being an accessible, customer focused and a knowledgeable point of contact. To provide a consistent approach across the Trust, Standard Operating Procedures (SOPs) will describe how the functions of the role will be carried out, along with the timescales to which are to be adhered. The potential candidate will be responsible for leading the Trust on the management of the patient tracking list for all patients on an active 18 week pathway, ensure 2 week wait targets are met and all other local and national targets.

You will also be expected to demonstrate a high level of prioritising, organisational and communication skills.

Qualifications

  • GCSE A Level or equivalent standard of Education
  • Supervisory or office management skills equivalent to certificate level

Experience

  • Minimum of 3 years medical secretarial/pathway administration experience

  • Proactive with the ability to use own initiative
  • Able to present a positive image of her/himself and the service
  • Ability to self reflect, to carry out tasks of own role and identify what s/he needs to learn to able to do current job better
  • Ability to take an active role in agreed learning activities and keep a record of them
  • Knowledge of national targets with respect to the patient pathway. A sound knowledge and demonstrable competence re 18 week referral to treatment rules
  • Ability to use and maintain resources efficiently and effectively and encourage others to do so.
  • Acts responsibly as a team member and seeks help if necessary
  • A proven record of being able to work within set timeframes working to priorities and deadlines
  • Able to monitor quality of own work and others
  • Ability to produce and present written reports and correspondence of a high calibre
  • Competence with a range of IT packages, e.g. CRS and EPR
  • Methodical and logical approach to work prioritisation and planning
  • To have a sound understanding of NHS wide initiatives and experience of responding positively to ensure local implementation
  • Ability to provide constructive opinions and views on ideas for service improvement
  • Flexible approach to the planning and coordination of patient pathways to ensure maximum use of clinical time
  • The ability to plan and co-ordinate activities and changes to systems or standards

Knowledge

  • AMSPAR or equivalent e.g. a good level of medical terminology
  • ECDL or equivalent experience e.g. experience of a range of IT packages such as word, excel, outlook and power point
  • Awareness of patient pathways
  • Understanding of patient priorities and performance targets
  • Knowledge of healthcare administrative systems and processes

Communication

  • Highly developed communication skills with experience of dealing with patient’s anxiety in a reassuring manner. An ability to communicate professionally at all levels
