Job summary

ST MICHAEL'S HOSPICE (INCORPORATING THE FREDA PEARCE FOUNDATION) is seeking compassionate Social Care staff to join the Social Work, Information and Advice Team in Herefordshire. You will work alongside social workers, acting as the first point of contact for patients and families, offering practical advice and signposting across community services.

You will help patients and carers access benefits and grants, coordinate referrals, and liaise with partner agencies while maintaining confidential

Responsibilities

• Be the first point of contact for patients and families, providing practical advice and signposting to local services.
• Assist patients, carers and families with applications for benefits, grants and other assistance; coordinate referrals and liaise with partner agencies.
• Maintain confidentiality and manage records in line with GDPR and hospice policies.
• Develop knowledge of local community resources, referral pathways and available services to support independence and wellbeing.
• Contribute to a collaborative multidisciplinary team and support safeguarding processes as needed.
