Job summary

St Michael's Hospice in Hereford is seeking a Senior Palliative Care Support Worker to join the Community Services team. You will deliver high quality, individualized palliative care and support to patients and their families, while assisting our nursing team and MDT colleagues.

Shifts are 0800-1400, 1600-2100, 2200-0700 with rota patterns across 7 days, including weekends and holidays. DBS enhanced check is required for this post.

• Proven ability to communicate effectively with patients and families.
• Ability to work within a multidisciplinary team to deliver care.
• Commitment to compassionate, patient-centered palliative care.

Responsibilities

• Provide hands-on palliative care as part of the Community Services team.
• Support the nursing team and coordinate with other care professionals.
• Manage rotating shifts across days, nights, weekends and bank holidays.

You will be part of a large multidisciplinary team providing high quality, individualised, hands on palliative care to our patients in our Community Services team. As a Senior Palliative Care Support Worker, you will be able to provide the best possible care for our patients and you will provide support to our nursing team. The position will be located in our Community Services team, which is open 7 days a week covering 24 hour care. Shifts will be rotational, to include days/nights/weekends/bank holidays., The role of the Senior Palliative Care Support Worker is to ensure that St Michael's is able to continue to deliver high quality palliative care and support to our patients and their families. This role requires effective communication, and the ability to establish supportive and working relationships with our patients and families as well as our Multidisciplinary team. Shift times are 0800-1400, 1600-2100, 2200-0700 This post is conditional to an enhanced Disclosure and Barring Service (DBS) Check.
