Job summary

Change Grow Live is seeking a Partnership Lead to develop robust partnership pathways and sustain a service integration strategy across dependency and recovery networks in South Yorkshire. The role focuses on collaboration between internal teams and external partners to improve care and treatment experiences for service users.

You will lead cross-sector work, empowering staff and partners while aligning local services with health and social care strategies.

Qualifications

• Background in substance misuse services, criminal justice services, or working with vulnerable adults in health and social care.
• Strong project management abilities, excellent communication skills, and a solution-focused approach to challenges.
• Understanding of Health, Social Care, and Voluntary sector pathways, with a data-driven approach to influencing service provision.
• Commitment to CGL's values, with the ability to inspire and lead a diverse team.

Responsibilities

• Lead and integrate: Drive activity within probation and criminal justice networks, developing strong partnerships to support the South Yorkshire Wide Service.
• Collaborate and innovate: Work closely with services and partners to enhance collaboration and improve care and treatment experiences for our service users.
• Support and guide: Provide valuable information, advice, and guidance to services and partners, ensuring we are well-positioned to serve local systems and communities.
• Improve and evidence: Help CGL services explore and evidence how closely they integrate with criminal justice, aiming to align care with local needs, reduce drug and alcohol harms, and decrease reoffending rates.

Purpose of the Role:

Are you passionate about transforming lives? As a Team Leader at CGL, you will play a key role in developing robust partnership pathways and processes. You will lead and sustain a service integration strategy, fostering greater collaboration between internal and external partners within Dependency and Recovery. This role is crucial in accelerating our progress in meeting health and social care challenges and responding to the Drug Strategy.

Where: South Yorkshire

Hours: Full Time. 37.5 per week

Contract Type: Permanent

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Please note for this role you will need an enhanced DBS and MOJ Clearance.
