Qualifications

  • Experience or willingness to work with individuals with diverse and complex needs.

  • Understanding of person-centred care and proactive engagement.
  • Flexible and reliable approach to working patterns and client needs.
  • Previous care experience within supported living or daycare setting required
  • Strong conversational and engagement skills
  • Safeguarding experience
  • Understanding of Microsoft Office and IT systems
  • Knowledge of CQC Fundamental Standards and inspection expectations.
  • Experience supporting medication processes and appointments.
  • NVQ/QCF Level 2 or 3 in Health & Social Care (or willingness to work towards).
  • Full UK Drivers Licence

Summary:

Person-Centred Support & Engagement
  • Provide personalised support based on each individual’s needs and abilities.
  • Promote dignity, privacy, choice, independence, confidence and life skills.
  • Engage positively and meaningfully with clients at all times.
  • Read and follow all care plans and BIRDIE documentation before providing support.
  • Build positive relationships by talking with clients and understanding their needs.
  • Drive for business when required and provide all necessary documentation.
Health, Medication & Appointments
  • Support medication administration, ordering and stock checks in line with training and procedures.
  • Support clients to attend health, social care and community appointments.
  • Ensure personal care supplies and appropriate toiletries are always available.
  • Monitor and promptly report changes in health, wellbeing or behaviour.
Safeguarding, Quality & Compliance
  • Work in accordance with CQC Fundamental Standards and the Key Questions: Safe, Effective, Caring, Responsive and Well-Led.
  • Follow safeguarding procedures and report concerns without delay.
  • Follow care plans, risk assessments, behaviour support plans and clinical guidance.
  • Report changes or required updates to care plans to House Leads and Team Leaders.
  • Maintain confidentiality and comply with GDPR requirements.
Documentation & Reporting
  • Complete accurate, detailed and timely records using BIRDIE.
  • Record daily notes, outcomes, incidents, medication, body maps and reviews appropriately.
  • Keep records person-centred, factual, comprehensive and inspection-ready.
  • Keep Health Action Plans accurate and up to date.
  • Maintain work equipment and immediately report concerns to the Contracts Team.
  • Raise concerns on BIRDIE promptly and follow up with the Team Leader if there is no response within 24 hours.
  • Ensure notes accurately reflect the client’s day/night, interactions, behaviour, activities, conversations, medical concerns and any identified risks.
